My First Visit  

  So this is your first visit for you and/or your family. Here at Red Bank Dental Associates, we will need to know some very important information about you before we can take care of you.

• When you walk through the door see our friendly receptionist and she will help guide you through the steps necessary to start your visit
• The receptionist will have you fill out a Welcome form and the Acknowledgment of Receipt of Notice of Privacy Practices as well as the copy of the Notice of Privacy Practices form. When finished with the Welcome form and the Acknowledgment of Receipt of Notice of Privacy Practices form you will hand them back to the receptionist. The Notice of Privacy Practice form you can take with you to read later if you like.
• Then you are ready for your visit with our Dentists.

If you are already a patient but your information has changed, you will need to fill out a new Welcome Form so that we can update your file so that we can make sure to give you the best and proper care.
